Handout – CPR SDM Agreement Form Instructions for Massachusetts Residents

The Center for Public Representation wants families and people with disabilities in Massachusetts to know about their options for decision-making support in adulthood, so they can find the right fit for them.  We partnered on a training initiative with the Cambodian Mutual Assistance Association in Lowell, Massachusetts.  This is the English version of one of the handouts we used. Hasan’s Journey with Supported Decision-Making

This is a video of a young man with disabilities, Hasan Ko, sharing his story with using Supported Decision-Making. In Mr. Ko’s culture, guardianship was not something that was used, and so his parents did not know what it was when Mr. Ko’s teachers said he needed a guardian. Mr. Ko talks about how he and his family decided to use Supported Decision-Making instead. This is just one example of how someone might use Supported Decision-Making.  Everyone’s path is different!

INFORME – Abordar la diversidad, la equidad y la inclusión:  Un proyecto para avanzar en la toma de decisiones con apoyo a través de asociaciones comunitaria (July 2024)

Desde julio de 2023 hasta junio de 2024, el Centro para la Representación Pública (CPR), en colaboración con socios clave de la comunidad y con el apoyo financiero del Consejo de Discapacidades del Desarrollo de Massachusetts (MDDC), diseñó e implementó una innovadora iniciativa de “llenado de vacíos” para hacer que la Toma de Decisiones con Apoyo (SDM) esté más disponible para las comunidades lingüística, étnica y culturalmente diversas en Massachusetts. Report – Addressing Diversity, Equity, & Inclusion: A Project to Advance Supported Decision-Making Through Community Partnerships (July 31, 2024)

From July 2023 through June 2024, the Center for Public Representation (CPR), in collaboration with key community partners and with the financial support of the Massachusetts Developmental Disabilities Council (MDDC), designed and implemented an innovative “gap-filling” initiative to make Supported Decision-Making (SDM) more available to linguistically, ethnically, and culturally diverse communities in Massachusetts. This final report summarizes the project, its approach, lessons learned, and promising practices. The report is also available in Spanish.

This Project, “Supported Decision-Making: Diversity, Equity & Inclusion,” was funded in part through a grant from the Massachusetts Developmental Disabilities Council, U.S. Department of Health and Human Services, Administration for Community Living, Federal Award No. 93.630.
